CCM is a service offered by our office that provides a PROACTIVE approach to keep you out of the hospital longer and provide guidance in managing your chronic conditions so you can be as healthy as possible. We develop an individualized plan of care based on your diagnoses and needs. This service includes a minimum of a monthly phone call from our nurse. It is similar to your doctors visit as we will review your medications and any questions about testing. This also provides us an opportunity to review any symptoms/problems you may be having. This is a direct link between the nurse who is calling you and the healthcare provider.  CCM is offered in addition to your regular check-ups and appointments.
